bulletWhat is your training, and how long have you been practicing?

bulletAre you licensed or certified? (Psychiatrists have an M.D. or D.O. {medical degree}, PhD's are licensed psychologists; Therapists in Washington are certified (tested and screened) or registered (available to any counselor).

bulletHave you worked with these specific issues before? (after describing your issues briefly).

bulletWhat are your fees and how do you expect payment?

bulletAre you covered by my insurance? Will there be a co-pay?

bulletWhen do you expect payment?

bulletWhat is your appointment cancellation policy?

bulletWhen is your first opening?

bulletConfidentiality: Therapy is confidential, absolutely and completely, unless there is danger to yourself or to others, including victimization of a child or incompetent adult. In those cases, the therapist has the legal duty to report to the appropriate government agency. Please ask your therapist to explain confidentiality in your therapy relationship.
