I have been receiving a huge volume of spam generated from my online sign up form, so I am now asking that you send me an email with your information to mcnicholsterry (at) hotmail.com with your first and last name, your degree or title, group or agency affiliation, address, city, state, zip code, phone and fax numbers, email address and specialties.
